Health Coach (0.8 FTE)

Nelson Bays Primary Health (NBPH) is the Primary Health Organisation (PHO) for the Nelson Tasman region. PHO’s lead and co-ordinate primary health care within the local community.  NBPH also manages an integrated health facility, Golden Bay Community Health, who deliver a wide range of health services to the local rural community in Takaka.

An exciting opportunity has become available for the following role:

 

Health Coach

0.8 FTE (64 hours per fortnight)

Te Tumu Waiora is an innovative model of Integrated Primary Mental Health and Addictions (IPMHA) services aimed to reduce barriers to accessing quality, convenient and patient-centred care. The Health Coach role is a diverse and exciting position that offers an impactful range of wellness support within general practices. You will be working in partnership with people who need support for behavioural or lifestyle changes, promoting self-management and achievement of goals and helping them navigate and connect with any necessary partner services that will help them succeed.

Working as a Health Coach offers a busy but incredibly rewarding environment, where you will tangibly see the difference and positive impact you can have on people’s lives. Training and support will be provided for all successful applicants.

This role will be based across two practices, Mapua and Wakefield. The role is Monday to Thursday, 8:30am to 5:00pm.

You will:

  • Work across the lifespan.

  • Work within and a be a resource person to the general practice team.

  • Have strong cultural awareness to support our diverse community.
