Healthy Lifestyle Facilitator (0.5 FTE)

Nelson Bays Primary Health (NBPH) is the Primary Health Organisation (PHO) for the Nelson Tasman region. PHO’s lead and co-ordinate primary health care within the local community. NBPH also manages an integrated health facility, Golden Bay Community Health, who deliver a wide range of health services to the local rural community in Takaka.

NBPH is looking for a community-focused Healthy Lifestyles Facilitator to join our Green Prescription team. You’ll support individuals to prevent and better manage long-term conditions by building confidence, knowledge and practical self-management skills that lead to sustainable behaviour change.

You’ll bring:

• Qualification or experience in community health, fitness or recreation

• Understanding of health promotion principles

• Skills in motivational interviewing and supporting self-management

• A passion for improving equity and wellbeing in our community
