More than a GP visit: Discover the support available through general practice

When was the last time you visited your GP?

Like most people, you were probably there to see a doctor or nurse. But you may not realise that your general practice can also connect you with a wide range of services designed to support your physical health, mental wellbeing, and overall quality of life.

Across Nelson Tasman, many of these services are available through general practice and supported by Nelson Bays Primary Health (NBPH), helping people get the right care, in the right place, at the right time.

Feeling overwhelmed, stressed, anxious or struggling with life's challenges? Support may be available through Health Improvement Practitioners, Health Coaches, counselling services, and alcohol and other drug support programmes. These services work alongside your GP team, providing practical help when and where it's needed.

Looking to become more active or improve your wellbeing? Rongoā Kākāriki (Green Prescription) helps people make sustainable lifestyle changes through physical activity, healthy eating and personal wellbeing goals. Sometimes a little support is all that's needed to get started.

For people living with long-term conditions, there is also a range of specialist support available. Services include respiratory education, pulmonary rehabilitation, cardiac rehabilitation, diabetes support, podiatry services and programmes that help people manage their health with confidence.

As we age, staying active and independent becomes increasingly important. Falls Prevention programmes provide practical advice and exercise support to help reduce the risk of falls, maintain mobility and build confidence.

Another lesser-known service is Advance Care Planning (ACP). ACP supports people to think ahead and discuss what matters most to them should they ever be unable to make decisions about their healthcare. Having these conversations early can provide reassurance for both individuals and their whānau.

For some people, simply navigating the health system can be challenging. Through services such as Kaiatawhai and Locality Care Coordination, people can receive support to access appointments, understand available services, and connect with the healthcare and community support they need. These services help break down barriers and ensure no one has to navigate their health journey alone.

The support available through general practice extends well beyond the consultation room. Programmes are designed to help people stay healthy, recover well, manage ongoing health conditions and improve their overall wellbeing.

Many local residents will also be familiar with the Medical & Injury Centre, which provides urgent and after-hours care when a visit to the Emergency Department may not be necessary.

Importantly, local people have a voice in shaping these services through the Health Consumer Advisory Group, ensuring community perspectives remain at the heart of healthcare planning.
